Судомний синдром на догоспітальному етапі: клінічні виклики, типові помилки та сучасні доказові підходи

Резюме

Актуальність. Судомний синдром належить до найпоширеніших невідкладних неврологічних станів, з якими фахівці екстреної медичної допомоги стикаються на догоспітальному етапі. Несвоєчасне розпізнавання причин судом, помилки у виборі тактики лікування або затримка з введенням протисудомних препаратів можуть призводити до розвитку епілептичного статусу (status epilepticus), вторинного ушкодження головного мозку, дихальної недостатності та погіршення клінічних результатів. Мета: узагальнити сучасні доказові підходи до діагностики та невідкладної допомоги при судомному синдромі на догоспітальному етапі, проаналізувати типові помилки під час надання допомоги та продемонструвати клінічне значення своєчасного виявлення ургентної неврологічної патології на основі клінічних випадків. Матеріали та методи. Проведено наративний огляд наукової літератури з використанням бази даних PubMed та сучасних міжнародних рекомендацій щодо менеджменту судом і status epilepticus на догоспітальному етапі. Проаналізовано дані щодо ефективності бензодіазепінів як препаратів першої лінії, особливостей первинної оцінки пацієнтів і типових помилок під час надання невідкладної допомоги. Додатково наведено клінічні випадки судомного синдрому, асоційованого із субарахноїдальним крововиливом, метастатичним ураженням головного мозку, тяжкою черепно-мозковою травмою та фармакорезистентною епілепсією. Результати. Аналіз літератури підтвердив, що основою ефективного менеджменту судом є раннє розпізнавання нападу, оцінка життєво важливих функцій за алгоритмом ABCDE, своєчасне введення бензодіазепінів і виявлення потенційно оборотних причин судомного синдрому. Найчастішими помилками залишаються затримка медикаментозного лікування, недооцінка тривалості судом, порушення принципів забезпечення прохідності дихальних шляхів і недостатня увага до етіології нападів. Наведені клінічні випадки демонструють, що судоми можуть бути першим проявом тяжкої нейрохірургічної або онкологічної патології та потребують невідкладної маршрутизації пацієнта до спеціалізованого стаціонару. Висновки. Судомний синдром на догоспітальному етапі потребує швидких і доказово обґрунтованих клінічних рішень. Використання стандартизованих алгоритмів, раннє застосування бензодіазепінів та своєчасне розпізнавання ургентної неврологічної патології сприяють поліпшенню результатів лікування та зменшенню ризику розвитку тяжких ускладнень.

Background. Seizure syndrome is one of the most common neurological emergencies encountered by emergency medical service providers in the prehospital setting. Delayed re­cognition of seizure etiology, inappropriate management strategies, or untimely administration of anticonvulsant therapy may result in status epilepticus, secondary brain injury, respiratory failure, and poor clinical outcomes. The purpose was to summarize current evidence-based approaches to the diagnosis and emergency care of seizure syndrome in the prehospital setting, to analyze common management errors, and demonstrate the clinical importance of timely recognition of urgent neurological pathology using clinical case examples. Materials and methods. A narrative literature review was conducted using publications indexed in the PubMed database and contemporary international guidelines on the management of seizures and status epilepticus in the prehospital setting. Evidence regarding the effectiveness of benzodiazepines as first-line therapy, features of initial patient assessment, and common errors when providing emergency care was analyzed. In addition, clinical cases of seizure syndrome associated with subarachnoid hemorrhage, metastatic brain disease, severe traumatic brain injury, and drug-resistant epilepsy were reviewed. Results. The literature analysis confirmed that effective seizure management is based on early seizure recognition, assessment of vital functions according to the ABCDE approach, timely administration of benzodiazepines, and identification of potentially reversible causes of seizures. The most common management errors include delayed pharmacological treatment, underestimation of seizure duration, inadequate airway management, and insufficient attention to the underlying etiology of seizures. The presented clinical cases demonstrate that seizures may be the first manifestation of severe neurosurgical or oncological pathology and require urgent referral to a specia­lized medical center. Conclusions. Seizure syndrome in the prehospital setting requires rapid and evidence-based clinical decision-making. The use of standardized management algorithms, early administration of benzodiazepines, and timely recognition of urgent neurological conditions contribute to improved patient outcomes and reduced risk of severe complications.
