Оборотна деменція в молодого пацієнта: випадок нейросифілісу, що імітує психіатричне захворювання

Актуальність. Нейросифіліс залишається складним для діагностики захворюванням через здатність імітувати первинні психіатричні та нейродегенеративні розлади, що часто призводить до затримки або помилок у встановленні діагнозу. Водночас він є рідкісною, але потенційно оборотною причиною зниження когнітивних функцій у відносно молодих пацієнтів, що визначає критичну важливість раннього виявлення. Клінічний випадок. Описано 44-річного чоловіка з когнітивним зниженням, яке прогресувало протягом одного року, проявлялося порушенням пам’яті, змінами особистості й нетриманням сечі. Спочатку пацієнт був госпіталізований до психіатричного стаціонару з діагнозом тяжкого депресивного розладу з когнітивним дефіцитом (псевдодеменція). Незважаючи на лікування антидепресантами, антипсихотиками, ноотропними препаратами та проведення електросудомної терапії, клінічного поліпшення не спостерігалося. Неврологічне обстеження виявило дезорієнтацію, виражені порушення короткочасної пам’яті, виконавчу дисфункцію та відсутність критики до свого стану. Магнітно-резонансна томографія головного мозку не продемонструвала структурних змін. Хоча експрес-тест на сифіліс (RPR) був негативним, трепонемоспецифічний імуноаналіз виявився позитивним. Дослідження спинномозкової рідини показало лімфоцитарний плейоцитоз при негативному RPR ліквору, що відповідало пізній стадії нейросифілісу (прогресивний параліч). Серологічні тести на хворобу Лайма були негативними, а інші причини деменції молодого віку, як-от ВІЛ-інфекція, хвороба Альцгеймера й лобово-скронева деменція, були виключені. Пацієнт отримав 14-денний курс високодозової внутрішньовенної терапії пеніциліном G. Після лікування спостерігалося значне клінічне поліпшення: відновлення орієнтації, часткове відновлення пам’яті й емоційної сфери, а також повернення здатності до самостійного функціонування. Висновки. Наведений випадок підкреслює актуальність нейросифілісу в диференціальній діагностиці швидко прогресуючого когнітивного зниження й атипових психіатричних проявів у молодих пацієнтів. Негативний нетрепонемний тест не виключає нейросифіліс, особливо на пізніх стадіях; встановлення діагнозу потребує використання трепонемоспецифічних тестів і, нерідко, дослідження ліквору. З огляду на потенційну оборотність цього стану своєчасна діагностика й лікування мають вирішальне значення. Рутинний скринінг на сифіліс та мультидисциплінарний підхід слід розглядати у випадках незрозумілого когнітивного дефіциту в осіб молодого віку.

Background. Neurosyphilis remains a diagnostically challenging condition due to its ability to mimic primary psychiatric and neurodegenerative disorders. This often results in delayed or incorrect diagnosis. Importantly, it represents a rare but potentially reversible cause of cognitive decline in relatively young patients, making early recognition clinically critical. Case presentation. We describe a 44-year-old man who developed progressive cognitive decline over one year, characterized by memory impairment, personality changes, and urinary incontinence. He was initially admitted to a psychiatric unit and diagnosed with severe depressive disorder with cognitive impairment (pseudodementia). Despite treatment with antidepressants, antipsychotics, nootropic agents, and electroconvulsive therapy, no clinical improvement was observed. Neurological evaluation revealed disorientation, profound short-term memory deficits, executive dysfunction, and impaired insight. Brain magnetic resonance imaging showed no structural abnormalities. Although the serum rapid plasma reagin (RPR) test was non-reactive, a treponemal-specific immunoassay was positive. Cerebrospinal fluid (CSF) analysis demonstrated lymphocytic pleocytosis with a non-reactive CSF-RPR, consistent with late-stage neurosyphilis (general paresis). Serologic testing for Lyme disease was negative, and alternative causes of early-onset dementia such as HIV infection, Alzheimer disease, and frontotemporal dementia were excluded. The patient received a 14-day course of high-dose intravenous penicillin G. Following treatment, he exhibited substantial clinical improvement, including restoration of orientation, partial recovery of memory and affect, and regained functional independence. Conclusions. This case highlights the continued relevance of neurosyphilis in the differential diagnosis of rapidly progressive cognitive decline and atypical psychiatric presentations in younger adults. A negative non-treponemal test does not exclude neurosyphilis, particularly in late stages; diagnosis requires treponemal-specific assays and, frequently, CSF examination. Given its potential reversibility, timely identification and treatment are essential. Routine screening for syphilis and a multidisciplinary diagnostic approach should be considered in cases of unexplained cognitive impairment in younger patients.
