Серцева недостатність та метаболічний синдром: роль інгібіторів натрійзалежного котранспортера глюкози 2-го типу у клінічний практиці

Резюме

Серцева недостатність (СН) та метаболічний синдром (МС) є двома взаємопов’язаними глобальними проблемами сучасної медицини. Основними факторами ризику розвитку хронічної СН є такі компоненти МС: вісцеральне ожиріння, інсулінорезистентність, гіперінсулінемія, дисліпідемія, артеріальна гіпертензія (АГ) та цукровий діабет (ЦД) 2-го типу. У даному огляді проаналізовано сучасні дані щодо взаємозв’язку між МС та СН, особливостей діагностики та ведення таких пацієнтів, що включає модифікацію способу життя, корекцію супутніх станів та оптимізацію лікування самої СН. Результати масштабних клінічних досліджень, зокрема DAPA-HF та EMPEROR-Reduced, довели ефективність інгібіторів натрійзалежного котранспортера глюкози 2-го типу (іНЗКТГ-2) для полегшення перебігу СН зі зниженою фракцією викиду (СНзнФВ) незалежно від наявності ЦД. Зокрема, дослідження DAPA-HF показало зниження ризику планової госпіталізації з приводу погіршення СН на 30 %, зменшення ризику ургентної госпіталізації та серцево-судинної смерті відповідно на 57 та 18 %. Подальші клінічні дослідження EMPEROR-Preserved та DELIVER продемонстрували ефективність емпагліфлозину та дапагліфлозину у зниженні ризику госпіталізацій з приводу СН також і для пацієнтів із СН зі збереженою фракцією викиду (СНзбФВ). Дослідження EMPEROR-Preserved виключно оцінило ефективність іНЗКТГ-2 (емпагліфлозину) у пацієнтів із СН з помірно зниженою ФВ (СНпзнФВ) та СНзбФВ незалежно від наявності діабету. Порівняно з плацебо емпагліфлозин знижував ризик первинної кінцевої точки, яка включала серцево-судинну смерть або гостру СН, на 21 %, переважно за рахунок зниження ризику розвитку гострої СН. Метааналіз даних досліджень DELIVER і EMPEROR-Preserved підтвердив, що в досліджуваній популяції іНЗКТГ-2 значно знижують як ризик комбінації серцево-судинної смерті та розвитку гострої СН, так і ризики окремих її компонентів. Як наслідок, застосування іНЗКТГ-2 емпагліфлозину або дапагліфлозину в пацієнтів із ЦД 2-го типу і СНзбФВ лівого шлуночка > 40 % рекомендується для зниження ризику гострої СН і серцево-судинної смерті й розглядається в настанові ESC як рекомендація класу ІА.

Heart failure (HF) and metabolic syndrome (MetS) are two interconnected global challenges in modern medicine. The main risk factors for the development of chronic HF include the following components of MetS: visceral obesity, insulin resistance, hyperinsulinemia, dyslipidemia, hypertension, and type 2 diabetes mellitus (T2DM). This review analyzes current data on the relationship between MetS and HF, as well as the diagnostic and management features in such patients, which include lifestyle modification, correction of concomitant conditions, and optimization of HF treatment itself. The results of large-scale clinical trials, particularly DAPA-HF and EMPEROR-Reduced, have demonstrated the efficacy of sodium-glucose cotransporter-2 inhibitors (SGLT2i) in improving the course of heart failure with reduced ejection fraction, regardless of the presence of T2DM. Specifically, the DAPA-HF trial showed a 30 % reduction in the risk of planned hospitalization for worsening HF, and a 57 and 18 % reduction in the risk of urgent hospitalization and cardiovascular death, respectively. Subsequent clinical trials, EMPEROR-Preserved and DELIVER, demonstrated the efficacy of empagliflozin and dapagliflozin in reducing the risk of HF hospitalizations in patients with heart failure with preserved ejection fraction (HFpEF) as well. The EMPEROR-Preserved trial specifically evaluated the efficacy of SGLT2i (empagliflozin) in patients with HF with mildly reduced ejection fraction and HFpEF, regardless of diabetes status. Compared to placebo, empagliflozin reduced the risk of the primary composite endpoint, which included cardiovascular death or acute HF, by 21 % — an effect mainly driven by a reduction in the risk of acute HF. A meta-analysis of the DELIVER and EMPEROR-Preserved trials confirmed that in the studied population, SGLT2i significantly reduced the risk of the composite of cardiovascular death and development of acute HF, as well as the risks of its individual components. Consequently, the use of SGLT2i, empagliflozin or dapagliflozin, in patients with T2DM and HFpEF > 40 % is recommended to reduce the risk of acute HF and cardiovascular death and is considered a class IA recommendation in the ESC guidelines.
