Передопераційна оцінка мозкового притоку як предиктор церебральної десатурації при артроскопіях плечового суглоба

Актуальність. Напівсидяче положення під час артроскопії плечового суглоба асоціюється зі змінами системної та церебральної гемодинаміки, що може призводити до розвитку церебральної гіпоперфузії та інтраопераційної десатурації. Водночас можливість прогнозування церебральної десатурації на основі передопераційної оцінки мозкового притоку залишається недостатньо вивченою. Мета: оцінити взаємозв’язок між передопераційними показниками глобального мозкового кровотоку, визначеного як сумарний об’ємний кровотік у внутрішній сонній артерії та хребтовій артерії, та інтраопераційними змінами церебральної оксигенації за даними near-infrared spectroscopy (NIRS) у пацієнтів у напівсидячому положенні. Матеріали та методи. У дослідження включено 48 пацієнтів, яким виконували артроскопію плечового суглоба в напівсидячому положенні. Передопераційно проводили ультразвукову оцінку однобічного мозкового притоку шляхом визначення сумарного об’ємного кровотоку у внутрішній сонній і хребтовій артеріях, а також моніторинг церебральної оксигенації методом near-infrared spectroscopy. Оцінювали показники в положенні лежачи та при моделюванні напівсидячого положення у свідомості. Інтраопераційно здійснювали безперервний NIRS-моніторинг, контроль артеріального тиску та частоти серцевих скорочень. Когнітивні функції оцінювали за шкалою MoCA до та після оперативного втручання. Результати. У пацієнтів зі зниженим передопераційним мозковим притоком відзначалося більш виражене зниження церебральної оксигенації як під час позиційної проби у свідомості, так і в інтраопераційному періоді. Інтраопераційні показники rSO2 у ІІ групі становили 82,4 ± 6,8 % проти 92,13 ± 2,69 % у І групі. Одночасно спостерігалася тенденція до зниження показників артеріального тиску й підвищення частоти серцевих скорочень. У пацієнтів зі зниженим мозковим притоком також виявлено тенденцію до помірного післяопераційного зниження когнітивних функцій за шкалою MoCA. Висновки. Передопераційна оцінка мозкового притоку може бути корисною для виявлення пацієнтів з підвищеним ризиком інтраопераційної церебральної десатурації під час артроскопії плечового суглоба в напівсидячому положенні. Знижений церебральний перфузійний резерв асоціюється з більш вираженим зниженням церебральної оксигенації та тенденцією до ранніх післяопераційних когнітивних змін.
Background. The beach-chair position during shoulder arthroscopy is associated with systemic and cerebral hemodynamic alterations that may contribute to cerebral hypoperfusion and intraoperative desaturation. However, the predictive value of preoperative cerebral blood flow assessment for intraoperative desaturation remains insufficiently investigated. The purpose was to evaluate the relationship between preoperative cerebral blood flow parameters defined as total volumetric flow in the internal carotid and vertebral arteries and intraoperative changes in cerebral oxygenation measured by near-infrared spectroscopy (NIRS) in the beach-chair position. Materials and methods. A total of 48 patients undergoing shoulder arthroscopy in the beach-chair position were included in the study. Preoperative unilateral cerebral blood flow was assessed using duplex ultrasonography by calculating total volumetric flow in the internal carotid and vertebral arteries, as well as monitoring cerebral oxygenation by NIRS. Measurements were obtained in the supine position and during simulated beach-chair positioning in awake patients. Continuous intraoperative NIRS monitoring, blood pressure, and heart rate assessment were performed. Cognitive function was evaluated using the Montreal Cognitive Assessment (MoCA) before and after surgery. Results. Patients with reduced preoperative cerebral blood flow demonstrated a more pronounced decrease in cerebral oxygenation both during preoperative positional testing and intraoperatively. Intraoperative rSO2 values in group II were 82.4 ± 6.8 % versus 92.13 ± 2.69 % in group I. Additionally, lower arterial pressure values and higher heart rates were observed. Patients with reduced cerebral perfusion also had a tendency toward mild postoperative cognitive decline according to MoCA scores. Conclusions. Preoperative assessment of cerebral blood flow may help identify patients at increased risk of intraoperative cerebral desaturation during shoulder arthroscopy in the beach-chair position. Reduced cerebral perfusion reserve was associated with a greater decrease in cerebral oxygenation and a tendency toward early postoperative cognitive impairment.
