Особливості вітреоретинальних втручань при ідіопатичних епіретинальних мембранах

Актуальність. Ідіопатичні епіретинальні мембрани (ЕRМ) належать до найбільш поширених захворювань вітреомакулярного інтерфейсу та є однією з провідних причин зниження центрального зору у пацієнтів старших вікових груп. За даними сучасних досліджень, поширеність ЕRМ становить від 7 до 39 %. З огляду на старіння населення кількість пацієнтів із симптоматичними ЕRМ постійно зростає, що визначає високу медико-соціальну значущість проблеми та необхідність удосконалення сучасних підходів до їх лікування. Мета: дослідити ефективність сучасних комбінових та двохетапних методів хірургічного лікування епіретинальних мембран у поєднанні з віковою катарактою. Матеріали та методи. Під спостереженням у дослідженні перебували 150 пацієнтів (150 очей) з ERM та початковою віковою катарактою, які були розподілені у 2 групи. Вік досліджуваних пацієнтів становив 61,0 ± 2,3 року. У 1-й групі (81 око) виконали закриту субтотальну вітректомію 25Ga (ЗСВ) з пілінгом ERM. Факоемульсифікацію катаракти (ФЕК) проводили через 3 місяці. У 2-й групі (69 очей) — ЗСВ з пілінгом ERM/внутрішньої пограничної мем­брани (ILM) з ФЕК та імплантацією інтраокулярної лінзи. Усім пацієнтам виконували оптичну когерентну томографію макулярної ділянки, за результатами якої визначали центральну товщину сітківки, оцінювали морфологічні зміни макулярної ділянки, наявність епімакулярної мембрани. Обстеження проводили до оперативного лікування, а також через 1, 3, 6 та 12 місяців після операції. Результати. Комбіноване виконання ФЕК під час вітреоретинального втручання супроводжується більшою частотою розвитку макулярного набряку у ранньому післяопераційному періоді. Водночас зазначені зміни були тимчасовими. Уже через три місяці їх частота суттєво зменшувалася, а через один рік клінічно значущий макулярний набряк був відсутній у всіх пацієнтів незалежно від виду оперативного лікування. Це свідчить про сприятливий довгостроковий прогноз після обох варіантів втручання. ­Висновки. Проведені нами дослідження показали, що ЗСВ з видаленням виключно ERM та ЗСВ з подвійним пілінгом ERM/ILM без хірургії та з хірургією катаракти є ефективними методами лікування пацієнтів з ERM, що сприяють поліпшенню зорових функцій при терміні спостереження 1 рік. ФЕК суттєво не поліпшувала гостроту зору, а, навпаки, була причиною резистентного макулярного набряку у терміни спостереження 1 та 3 місяці. Пілінг ILM не поліпшує функціональні результати після операції EРM, але запобігає виникненню рецидивів ERM.

Background. Idiopathic epiretinal membranes (ERM) are among the most common disorders of the vitreomacular interface and are one of the leading causes of reduced central vision in older patients. According to recent studies, the prevalence of ERM ranges from 7 to 39 %. With the aging of the population, the number of patients with symptomatic ERMs continues to increase, highligh­ting the high medical and social significance of this condition and the need to improve current treatments. Objective: to investigate the efficacy of contemporary combined and two-stage surgical approaches for the treatment of epiretinal membranes associated with age-related cataract. Materials and methods. The study included 150 patients (150 eyes) with ERM and initial age-related cataract, who were divided into two study groups. The mean age of the participants was 61.0 ± 2.3 years. In group 1 (81 eyes), 25-gauge closed subtotal vitrectomy (CSV) with ERM peeling was performed, followed by phacoemulsification 3 months later. In group 2 (69 eyes), CSV with combined ERM/internal limiting membrane (ILM) peeling was performed simultaneously with phacoemulsification and intraocular lens implantation. All patients underwent optical coherence tomo­graphy of the macular region, which was used to determine central retinal thickness, assess morphological changes in the macula, and detect the presence of an epiretinal membrane. Examinations were performed before surgery and at 1, 3, 6, and 12 months postoperatively. Results. Phacoemulsification in combination with vitreore­tinal surgery is associated with a higher incidence of macular edema in the early postoperative period. However, these changes were transient; their incidence decreased significantly after 3 months, and at one year, clinically significant macular edema was absent in all patients, regardless of the type of surgical intervention. This indicates a favorable long-term prognosis for both surgical approaches. Conclusions. Our studies demonstrated that CSV with ERM peeling only and CSV with combined ERM/ILM peeling, performed either without or with simultaneous cataract surgery, are effective methods for the treatment of ERM and result in improved visual function over a 1-year follow-up period. Phacoemulsification did not significantly improve visual acuity and, conversely, was associated with a higher incidence of persistent macular edema at 1 and 3 months postoperatively. ILM peeling did not improve functional outcomes after ERM surgery but reduced the risk of ERM recurrence.
