Особливості структури, ультраструктури капсули та епітелію кришталика при ускладненій катаракті

Актуальність. Вивчення властивостей кришталика з клінічної точки зору має велике значення в катарактальній хірургії. Подані результати дослідження структури й ультраструктури капсули та епітелію кришталика при катаракті, що поєднується з міопією середнього і високого ступеня та первинною відкритокутовою глаукомою. Мета: вивчити структурні та ультраструктурні особливості будови капсули та епітелію кришталика при віковій катаракті, катаракті в поєднанні з міопією середнього та високого ступеня і в поєднанні з первинною відкритокутовою глаукомою. Матеріали та методи. Проведено дослідження фрагментів передньої капсули кришталика 9 очей оперованих пацієнтів. Вік пацієнтів з ускладненою катарактою — від 57 до 83 років. Вік пацієнтів контрольної групи — від 63 до 78 років. Гістологічні й електронно-мікроскопічні дослідження включали визначення стану епітелію, клітин, товщин базальної мембрани, міжклітинного простору. Отримано й проаналізовано 127 електронних мікрофотографій. Результати. Встановлено, що багато клітин епітелію кришталика мають ознаки деструкції органел, є ділянки, зовсім позбавлені клітин. При віковій катаракті, що поєднується з міопією високого ступеня, у передній капсулі кришталика виявляються деструктивні зміни колагенових волокон різних ступенів вираженості з елементами набряку основної речовини, значна частина епітеліальних клітин знаходиться на стадії глибоких необоротних деструктивних змін. Висновки. Структура й ультраструктура капсули та епітелію кришталика при віковій катаракті й ускладненій катаракті різняться. У пацієнтів з ускладненою катарактою присутні ознаки пошкодження тканин за типом дегенеративно-дистрофічних, у пацієнтів з катарактою в поєднанні з міопією середнього та високого ступеня вони більш виражені. Ці особливості вимагають від офтальмохірурга особливого підходу при виборі тактики оперативного втручання і ведення післяопераційного періоду в пацієнтів з ускладненою катарактою.

Background. Studying the properties of the lens from a clinical perspective is of great importance in cataract surgery. This paper presents the results of a study on the structure and ultrastructure of the lens capsule and epithelium in cataract associated with moderate and high myopia and primary open-angle glaucoma. Objective: to investigate the structural and ultrastructural features of the lens capsule and epithelium in age-related cataract, cataract associated with moderate and high myopia, and cataract combined with primary open-angle glaucoma. Materials and methods. A study was conducted on fragments of the anterior lens capsule from 9 eyes of surgical patients. The age of patients with complicated cataract ranged from 57 to 83 years, while the age of the control group was 63 to 78 years. Histological and electron microscopic examinations included the assessment of the epithelial state, cellular morphology, basement membrane thickness, and intercellular spaces. A total of 127 electron micrographs were obtained and analyzed. Results. It was found that many lens epithelial cells contain organelles undergoing various degrees of destruction, with some areas being completely devoid of cells. In age-related cataract associated with high myopia, destructive changes in collagen fibers of varying severity were observed in the anterior lens capsule, accompanied by signs of ground substance edema. A significant portion of the epithelial cells exhibited profound, irreversible destructive changes. Conclusions. The structural and ultrastructural characteristics of the lens capsule and epithelium differ between age-related and complicated cataract. Patients with complicated cataract show signs of tissue damage consistent with degenerative and dystrophic changes, which are more pronounced in cases combined with moderate and high myopia. These features require a tailored approach from the ophthalmic surgeon when selecting surgical tactics and managing the postoperative period in patients with complicated cataract.
