Травматична оптична нейропатія при мінно-вибуховій травмі: особливості патогенезу зсувної деформації та клінічний випадок відновлення зорових функцій у дитини

Резюме

Актуальність. В умовах повномасштабної війни в Україні мінно-вибухова травма (МВТ) стала поширеною причиною тяжкої інвалідизації як військового, так і цивільного населення. Одним із найскладніших і прихованих ускладнень МВТ є травматична оптична нейропатія. Пошкодження внутрішньоканального сегмента зорового нерва та зсувні деформації (shear injury) на рівні решітчастої пластинки склери часто розвиваються за відсутності видимих анатомічних руйнувань очного яблука, що суттєво ускладнює своєчасну діагностику та погіршує зоровий прогноз. Мета: патофізіологічно обґрунтувати механізми первинного та вторинного контузійного пошкодження зорового нерва при МВТ (зокрема, компартмент-синдрому та зсувних деформацій аксонів) і продемонструвати регенераторний потенціал зорової системи у дитячому віці на прикладі успішної клінічної реабілітації. Клінічний випадок. Описано випадок успішного лікування 13-річного хлопчика з Харкова, який зазнав тяжкої контузії заднього полюса обох очей внаслідок МВТ. При первинному огляді на 4–5-ту добу гострота зору становила 0,1–0,3. Відзначалося концентричне звуження полів зору, масивний промінучий набряк дисків зорових нервів і перипапілярні геморагії на тлі збережених зіничних реакцій. За даними магнітно-резонансної та оптичної когерентної томографії верифіковано виражений ретробульбарний набряк без розриву волокон. Призначення комплексної патогенетичної терапії (системні кортикостероїди, нейро- та ретинопротектори) дозволило вивести аксони зі стану парабіозу. Через 2 місяці гострота зору відновилася до 0,8–0,9, поля зору повністю нормалізувалися. На очному дні сформувалася чітка зона конічної перипапілярної атрофії (1–2 мм) як патогномонічний слід перенесеної зсувної деформації тканин. Висновки. Контузійна МВТ зорового нерва вимагає негайної ургентної діагностики (комп’ютерна/оптична когерентна томографія) і диференційованого підходу. Дитяча зорова система має високу пластичність цитоскелета аксонів та еластичність решітчастої пластинки, що при своєчасному початку високодозної протинабрякової та нейропротекторної терапії забезпечує оборотність функціонального блоку (парабіозу) волокон і високий рівень відновлення зору.

Background. In the context of the ongoing full-scale war in Ukraine, blast injuries have become a prevalent cause of severe disability among both military personnel and the civilian population. Traumatic optic neuropathy represents one of the most challenging and occult complications of blast-induced trauma. Damage to the intracanalicular segment of the optic nerve and shear injuries at the level of the lamina cribrosa frequently develop in the absence of visible anatomical disruption of the eyeball, significantly complicating timely diagnosis and worsening the visual prognosis. Objective: to provide a pathophysiological rationale for the mecha­nisms of primary and secondary concussive optic nerve injury in blast trauma (specifically compartment syndrome and axonal shear injury) and to demonstrate the regenerative potential of the visual system in pediatric patients through a case of successful clinical rehabilitation. Case presentation. We report a case of successful mana­gement of a 13-year-old boy from Kharkiv who sustained a severe concussion of the posterior pole of both eyes due to a blast injury. At the initial examination on days 4–5, visual acuity was 0.1–0.3. A concentric visual field narrowing was detected, along with massive prominent edema of the optic discs, and peripapillary hemorrhages, while pupillary light reflexes remained intact. Magnetic resonance imaging and optical coherence tomography verified pronounced retrobulbar edema without axonal disruption. The comprehensive pathogenetic therapy (systemic corticosteroids, neuro- and reti­noprotectors) successfully reversed the axonal functional block (parabiosis). After 2 months, visual acuity improved to 0.8–0.9, and visual fields normalized completely. Fundus examination revealed a distinct zone of conoid peripapillary atrophy (1–2 mm), serving as a pathognomonic marker of the sustained tissue shear injury. Conclusions. Concussive blast injury to the optic nerve requires immediate urgent diagnostic evaluation (computed/optical coherence tomography) and a differentiated approach. Child’s visual system possesses high axonal cytoskeletal plasticity and lamina cribrosa elasticity. When high-dose anti-edema and neuroprotective therapy is initiated timely, it ensures the reversibility of the functional axonal block (parabiosis), leading to a high rate of visual recovery.
