Зоровий аналізатор при цукровому діабеті: результати ОКТ, електрофізіологічних досліджень та оцінки зіничного циклу

Резюме

Актуальність. Цукровий діабет супроводжується не лише мікросудинними змінами сітківки, а й розвитком нейродегенеративних порушень зорового аналізатора, які можуть виникати ще до появи клінічних ознак діабетичної ретинопатії. У звʼязку з цим особливого значення набуває застосування сучасних структурних і функціональних методів дослідження для раннього виявлення діабетичного ураження органа зору. Мета: оцінити можливості структурних та електрофізіологічних методів у ранній комплексній діагностиці порушень зорового аналізатора у пацієнтів із цукровим діабетом. Матеріали та методи. Обстежено 67 пацієнтів із цукровим діабетом 1-го та 2-го типу (134 ока), яких залежно від стадії діабетичної ретинопатії розподілено на три групи: з непроліферативною стадією (23 пацієнти, 46 очей), з препроліферативною стадією (22 пацієнти, 44 ока) та з проліферативною (22 пацієнти, 44 ока). Контрольну групу становили 20 практично здорових осіб (40 очей). Усім учасникам виконували комплексне офтальмологічне обстеження, що включало оптичну когерентну томографію (ОКТ) з визначенням товщини шару нервових волокон сітківки (RNFL), а також електрофізіологічні дослідження: визначення критичної частоти злиття миготінь (КЧЗМ), порогу електричної чутливості за фосфеном (ПЕЧФ), лабільності зорового аналізатора (ЛЗА) та часу зіничного циклу (ЧЗЦ). Статистичну обробку результатів проводили із використанням пакета MedStat v.5.2, статистично значущими вважали відмінності між групами при p < 0,05. Результати. У пацієнтів із цукровим діабетом встановлено прогресуюче погіршення структурних і функціональних показників зі збільшенням тяжкості діабетичної ретинопатії. За даними ОКТ відзначалося вірогідне зменшення товщини RNFL порівняно з контрольною групою. Одночасно виявлено зниження КЧЗМ і ЛЗА, підвищення ПЕЧФ та подовження ЧЗЦ, причому найбільш виражені зміни спостерігалися при проліферативній діабетичній ретинопатії. Отримані результати свідчать про поєднання структурних і функціональних проявів діабетичної нейродегенерації, які прогресують відповідно до стадії захворювання. Висновки. Поєднане застосування оптичної когерентної томографії та електрофізіологічних методів забезпечує комплексну оцінку структурного й функціонального стану зорового аналізатора у пацієнтів із цукровим діабетом. Зменшення товщини RNFL у поєднанні зі зниженням КЧЗМ та ЛЗА, підвищенням ПЕЧФ і подовженням ЧЗЦ є проявами діабетичної нейродегенерації та можуть використовуватися як інформативні маркери для ранньої діагностики, оцінки прогресування й моніторингу діабетичного ураження органа зору.

Background. Diabetes mellitus is associated not only with retinal microvascular alterations but also with neurodegenerative changes of the visual analyzer that may develop yet before the appearance of clinical signs of diabetic retinopathy. Therefore, the use of contemporary structural and functional assessment me­thods is of particular importance for the early detection of diabetic eye involvement. The purpose was to evaluate the diagnostic value of structural and electrophysiological methods for the early comprehensive assessment of visual analyzer impairment in patients with diabetes mellitus. Materials and methods. The study included 67 patients with types 1 and 2 diabetes mellitus (134 eyes), who were divided into three groups according to the stage of diabetic retinopathy: non-proliferative (23 patients, 46 eyes), pre-proliferative (22 patients, 44 eyes), and proliferative one (22 patients, 44 eyes). The control group consisted of 20 healthy subjects (40 eyes). All participants underwent a comprehensive ophthalmic examination, including optical coherence tomography with assessment of retinal nerve fiber layer (RNFL) thickness, as well as electrophysio­logical testing comprising critical flicker frequency (CFF), phosphene electrical sensitivity threshold (PEST), visual analyzer lability (VAL), and pupil cycle time (PCT). Statistical analysis was performed using MedStat v.5.2 software. Differences between groups were considered statistically significant at p < 0.05. Results. Patients with diabetes mellitus demonstrated progressive deterioration of both structural and functional parameters with increasing severity of diabetic retinopathy. Optical coherence tomography revealed a significant reduction in RNFL thickness compared to the control group. Simultaneously, CFF and VAL decreased, whereas PEST and PCT increased, with the most pronounced changes observed in patients with proliferative diabetic retinopathy. These findings indicate the coexistence of structu­ral and functional manifestations of diabetic neurodegeneration, which progress in parallel with disease severity. Conclusions. The combined application of optical coherence tomography and electrophysiological methods provides a comprehensive assessment of the structural and functional status of the visual analyzer in patients with diabetes mellitus. Decreased RNFL thickness toge­ther with reduced CFF and VAL, increased PEST, and prolonged PCT represent manifestations of diabetic neurodegeneration and may serve as informative biomarkers for the early diagnosis, assessment of disease progression, and monitoring of diabetic eye involvement.
