Вторинна глаукома як рідкісне ускладнення токсоплазмозного увеїту

Вторинна постувеальна глаукома є тяжким ускладненням увеїту, що потребує одночасного контролю внутрішньоочного тиску та активного запального процесу. У статті подано клінічний випадок діагностики й лікування вторинної увеальної глаукоми, асоційованої з очним токсоплазмозом, із критичним аналізом послідовності терапевтичних заходів. Мета: подати особливості діагностики та лікування вторинної постувеальної глаукоми на тлі очного токсоплазмозу, а також проаналізувати ефективність і можливі ризики застосованих методів контролю внутрішньоочного тиску. Матеріали та методи. Проведено аналіз клінічного випадку пацієнта з гострим увеїтом та вторинною увеальною глаукомою лівого ока. Оцінювали скарги, анамнез, гостроту зору, внутрішньоочний тиск, результати біомікроскопії, гоніоскопії, периметрії, ультразвукового дослідження та лабораторної діагностики TORCH-інфекцій. Проаналізовано динаміку захворювання на тлі медикаментозної терапії та хірургічних підходів. Результати. На початковому етапі захворювання було діагностовано відкритокутову глаукому лівого ока з підвищенням внутрішньоочного тиску до 43 мм рт.ст. з підозрою на запальний процес, якому не було приділено достатньо уваги. Надалі виявлено ознаки активного іридоцикліту, преципітати на ендотелії рогівки, неоваскуляризацію райдужки та гоніосинехії. Лабораторні дослідження допомогли у встановленні діагнозу очного токсоплазмозу. Аналіз та корекція медикаментозної терапії і правильно підібрана хірургічна тактика забезпечили зниження ВОТ до 10,5–13,0 мм рт.ст. у ранньому післяопераційному періоді. Через місяць внутрішньоочний тиск становив 33 мм рт.ст., гострота зору лівого ока поліпшилася до 0,6 без корекції. Подальший контроль стану пацієнта не здійснювався. Висновки. Вторинна постувеальна глаукома потребує комплексного підходу, що включає контроль внутрішньоочного тиску та активного запалення. Клінічна діагностика може бути складною через подібність проявів іншим формам вторинної глаукоми, зокрема синдрому Познера — Шлоссмана. Наведений випадок демонструє необхідність своєчасного уточнення етіології увеїту, обережного вибору лазерних та хірургічних методів лікування й динамічного спостереження за пацієнтами з високим ризиком прогресування глаукомної оптичної нейропатії.
Background. Secondary post-uveitic glaucoma is a severe complication of uveitis that requires simultaneous control of intraocular pressure and the active inflammatory process. This article presents a clinical case of the diagnosis and treatment of secondary uveitic glaucoma associated with ocular toxoplasmosis, with a critical analysis of the sequence of therapeutic interventions. The purpose was to present the diagnostic and therapeutic features of secondary post-uveitic glaucoma associated with ocular toxoplasmosis and to analyze the efficacy and potential risks of the methods used to control intraocular pressure. Materials and methods. A clinical case of a patient with acute uveitis and secondary uveitic glaucoma of the left eye was analyzed. Patient complaints, medical history, visual acuity, intraocular pressure, biomicroscopy, gonioscopy, perimetry, ultrasonography, and laboratory testing for TORCH infections were assessed. The course of the disease during medical treatment and surgical interventions was analyzed. Results. At the initial stage of the disease, open-angle glaucoma of the left eye was diagnosed, with intraocular pressure elevated to 43 mmHg and suspected inflammatory involvement, which had not received sufficient attention. Subsequently, signs of active iridocyclitis, corneal endothelial precipitates, iris neovascularization, and goniosynechiae were detected. Laboratory investigations contributed to the diagnosis of ocular toxoplasmosis. Analysis and adjustment of medical therapy, together with appropriately selected surgical management, resulted in a reduction in intraocular pressure to 10.5–13.0 mmHg during the early postoperative period. One month later, intraocular pressure was 33 mmHg, and visual acuity in the left eye improved to 0.6 without correction. No further follow-up was performed. Conclusions. Secondary post-uveitic glaucoma requires a comprehensive approach involving control of intraocular pressure and active inflammation. Clinical diagnosis may be challenging because of similarities with other forms of secondary glaucoma, particularly Posner-Schlossman syndrome. This case demonstrates the need for timely clarification of the etiology of uveitis, careful selection of laser and surgical treatment methods, and dynamic follow-up of patients at high risk of glaucomatous optic neuropathy progression.
