Застосування дапагліфлозину при лікуванні діабетичної міопатії: морфологічні аспекти ризиків і переваг

Актуальність. З огляду на частий зв’язок цукрового діабету і діабетичної міопатії та саркопенії вивчення впливу антидіабетичних препаратів на структуру і функцію скелетних м’язів має важливе клінічне значення. Враховуючи побоювання щодо здатності інгібіторів натрій-глюкозного котранспортера 2 спричиняти м’язову атрофію через катаболічні механізми, метою було оцінити переваги й обмеження монотерапії дапагліфлозином і комбінованого лікування стрептозотоцин-індукованого діабету (СЦД) шляхом морфологічного аналізу жувального м’яза. Матеріали та методи. У дослідженні використано 71 щура-самця лінії Вістар (180–200 г), розподілених на три групи: 1 — контрольну, 2 — групу нелікованого СЦД, 3 — групу терапії СЦД. Група 3 включала наступні підгрупи: 3а — інсулін гларгін/ліксенатид, 3б — дапагліфлозин, 3в — комбінована терапія інсуліном гларгіном/ліксенатидом і дапагліфлозином. Забір матеріалу здійснювали на 28-му і 56-ту добу експерименту в нелікованих тварин та на 14-ту й 42-гу — в терапевтичних групах. Результати. Монотерапія дапагліфлозином суттєво поліпшувала морфологічний стан жувального м’яза, у тому числі при тривалому застосуванні. Позитивні зміни супроводжувалися відновленням мікроциркуляції, збільшенням капілярної щільності й активацією внутрішньо- і позаклітинних регенераторних процесів. Однак, незважаючи на ці ефекти, площа м’язових волокон на 56-ту добу при використанні дапагліфлозину залишалася меншою, ніж у групі комбінованого лікування, хоча відмінності від монотерапії інсуліном не були статистично значущими. Ультраструктурний аналіз також продемонстрував зниження об’ємної щільності міофібрил після монотерапії дапагліфлозином. Висновки. Таким чином, дапагліфлозин сприяє регенераторним процесам у скелетних м’язах діабетичних щурів і поліпшує глікемічний контроль без ознак саркопенії. Проте монотерапія є недостатньою для повного структурного відновлення м’яза при тривалому діабеті, тоді як комбінована терапія (інсулін + дапагліфлозин) забезпечує більш виражені регенераторні та морфологічні ефекти у жувальному м’язі.

Background. Given the frequent association of diabetes mellitus with diabetic myopathy and sarcopenia, the effects of antidiabetic therapies on skeletal muscle structure and function remain clinically relevant. Considering concerns that sodium-glucose cotransporter 2 inhibitors may promote muscle atrophy through catabolic mechanisms, this study purposed to elucidate the advantages and disadvantages of using dapagliflozin monotherapy and in combination treatment of streptozotocin-induced diabetes mellitus (STZ-DM) via a morphological study of the masseter muscles. Materials and methods. Seventy-one male Wistar rats (180–200 g) were divided into three groups: 1 — control, 2 — with STZ-DM, and 3 — STZ-DM receiving treatment. The treatment group included the following subgroups: 3a — insulin glargine/lixisenatide; 3b — dapagliflozin monotherapy; 3c — combined therapy with insulin glargine/lixisenatide and dapagliflozin. Tissue sampling was performed on days 28 and 56 of the experiment in untreated animals and on days 14 and 42 in treated groups. Results. Dapagliflozin monotherapy markedly improved the morphological state of the masseter muscle, including under prolonged administration. Muscle recovery was associated with improved microcirculation, reflected by reduced vascular impairment and increased capillary density, alongside intracellular and extracellular regenerative changes. However, despite these beneficial effects, muscle fiber area at day 56 remained lower in the dapagliflozin group than in the combined-treatment group, although differences from insulin-based monotherapy were not statistically significant. Ultrastructural analysis also demonstrated reduced myofibrillar volume density after dapagliflozin monotherapy. Conclusions. Dapagliflozin promotes regenerative processes in diabetic skeletal muscle and improves glycemic control without inducing sarcopenia. Nevertheless, monotherapy appears insufficient for complete muscle recovery in prolonged diabetes, whereas combined insulin-dapagliflozin treatment provides superior regenerative and restorative outcomes in the masseter muscle.
