The review summarizes current approaches to anesthetic management of upper extremity surgery in the beach chair position. Although this position facilitates surgical access, it is associated with significant hemodynamic changes, including reduced venous return, cardiac output, and mean arterial pressure at the level of the brain, thereby increasing the risk of intraoperative hypotension and cerebral oxygen desaturation. Recommended strategies include gradual patient positioning, individualized adjustment of mean arterial pressure accounting for the hydrostatic gradient, the use of near-infrared spectroscopy for cerebral oxygenation monitoring, combined anesthesia techniques incorporating general and regional anesthesia, optimization of fluid management, and readiness for early vasopressor support. Evidence regarding the impact of preventive measures on long-term neurological outcomes remains limited, highlighting the need for standardized prospective multicenter studies.
