The article discusses the impact of alcohol and psychoactive substances on the central nervous system of adolescents with epilepsy, focusing on the risks of seizures and epileptic activity associated with their non-medical use. The phenomenon of amygdaloid kindling is examined as a universal model of neuronal sensitization that explains the effects of antiseizure medications in alcohol withdrawal syndrome, substance use disorders, and comorbid anxiety. The molecular mechanisms of pharmacotherapy are described, specifically the normalization of GABAergic and glutamatergic neurotransmission and the modulation of ion channels. The paper presents data from clinical and meta-analytic studies confirming the efficacy of valproates, carbamazepine, topiramate, gabapentin, pregabalin, and lamotrigine in managing withdrawal symptoms, reducing cravings for alcohol and psychoactive substances (such as cocaine and ketamine), preventing relapse, and treating co-occurring affective and anxiety disorders.
