Background. Bronchial obstruction syndrome is one of the most common clinical conditions in preschool children and frequently accompanies acute respiratory infections, episodes of viral-induced wheezing, and exacerbations of bronchial asthma. International clinical guidelines consider the combined use of short-acting bronchodilators, particularly β2-agonists and anticholinergic agents, as one of the options for symptomatic therapy of bronchial obstruction. Given the variability of clinical approaches, summarizing current recommendations on the use of ipratropium bromide and fenoterol in preschool children remains relevant. The purpose of the study was to summarize current scientific data on the efficacy and safety of inhaled bronchodilators, in particular anticholinergic agents (ipratropium bromide) used as monotherapy and in combination with a short-acting selective β2-agonist (fenoterol), in the treatment of bronchial obstructive syndrome in infants and preschool children. Materials and methods. A systematic analysis of scientific publications indexed in PubMed, Scopus, Web of Science, and the Cochrane Library was conducted for the period 2010–2026. The review included clinical studies, experimental research, and international consensus statements describing the use of ipratropium bromide and its combination with fenoterol hydrobromide in infants and preschool children with bronchial obstructive symptoms. Results. Inhalation therapy using ipratropium bromide or its combination with fenoterol hydrobromide represents a justified and effective approach to the symptomatic treatment of bronchial obstructive syndrome in young and preschool children. The combination of anticholinergic and β2-adrenergic mechanisms of action provides a pronounced bronchodilatory effect, targeting both vagally mediated bronchospasm and functional airway narrowing, which is particularly important in children with recurrent or severe episodes of bronchial obstruction. In clinical practice, inhalation therapy may include ipratropium bromide preparations, fixed combinations with fenoterol hydrobromide, which combine two bronchodilatory mechanisms. Clinical studies and international guidelines support the use of such a combination in cases of insufficient efficacy of monotherapy, as well as its favorable safety profile when administered via inhalation. Conclusions. Inhaled bronchodilators, including ipratropium bromide as monotherapy and in combination with fenoterol hydrobromide, are effective agents for the treatment of bronchial obstructive syndrome in infants and preschool children.
