Low-Frequency Ultrasound Therapy in Combination Treatment of Patients with Type 2 Diabetes Mellitus

Background. Hypoglycemic therapy used at present in patients with type 2 diabetes mellitus (DM) does not always lead to desired result and many patients live in a state of constant decompensation of metabolic processes, with the high levels of glycemia and glycosuria. That is why the search for the optimal treatment strategy that could increase the effectiveness of hypoglycemic therapy, and reduce the risk of complications is one of the most actual problems of modern diabetology.
The purpose of this study was to determine the use of low-frequency ultrasound therapy on cutaneous projection of the liver in patients with type 2 DM.
Materials and Methods. 30 patients with newly diagnosed type 2 DM and body mass index (BMI) greater than
25 kg/m2 were treated by the low-frequency ultrasound therapy. The ultrasonic effects were conducted in pulsed mode of 44 kHz and an amplitude of fluctuations — 2 microns using the apparatus MIT-11 as follows. Effects on the area of liver projection in patients was performed by the immovable technique (method), exposure to a session consisted of 8 minutes. The vaseline oil was used as a contact substance. Additionally influence on segmental area of Th7-Th12 by labile technique for 2 minutes on each plot was used to enhance the therapeutic effect. The patients were examined before and after treatment. The dynamics of clinical symptoms, glycemia and glycosuria, level of glycated hemoglobin was taken into account to assess the effectiveness of low-frequency ultrasound. To assess the degree of insulin resistance content of insulin, C-peptide and glucagon in blood plasma were determined by radioimmunoassay. Fasting insulin, C-peptide, glucagon in blood serum were tested in the morning. The statistical processing of tests’ results was performed using Student’s t test.
Results. The beginning of therapeutic effect was observed after 2 procedures of the ultrasound exposure. The maximum effect is appeared after 8–10 treatment sessions. The positive dynamics of complex treatment is improving the general state of health, a disappearance of asthenization, and a decrease in the symptoms of cardiovascular disorders, achieving faster compensation of carbohydrate metabolism. The course of treatment contributed to the hyperglycemia reduction in patients with newly detected type 2 DM. After ultrasound treatment, the authors noted a positive dynamics of clinical symptoms: an improvement of the general health status, a decrease in fatigue, an improvement of psycho-emotional indices, disappearance of pain in the right upper quadrant, and a decrease in liver size in all the patients under study.
Conclusions. The use of low-frequency ultrasound therapy on cutaneous projection of the liver in patients with type 2 DM promotes the normalization both fasting and postprandial glycemia. The effect of low-frequency ultrasound on cutaneous projection of the liver is significantly decreasing parameters that characterize the pancreatic insulin synthesizing function (immunoreactive insulin, C-peptide) in patients with newly diagnosed type 2 DM and a BMI > 25 kg/m2. Low-frequency ultrasound reduces the glucagon secretion and thereby positively affects the hepatic gluconeogenesis. Ultrasound therapy can be used in the complex treatment of patients with newly diagnosed type 2 DM.

Застосовувана сьогодні цукрознижувальна терапія хворих на цукровий діабет (ЦД) 2-го типу не завжди приводить до бажаного результату, і дуже багато хворих перебуває у стані постійної декомпенсації обмінних процесів із високими показниками рівня глікемії і глюкозурії. Тому пошук оптимальної тактики лікування, що могла б підвищити ефективність цукрознижувальної терапії, зменшити ризик розвитку ускладнень, залишається однією з найактуальніших проблем сучасної діабетології.
Мета дослідження — вивчити вплив низькочастотної ультразвукової терапії на шкірну проекцію печінки у хворих на ЦД 2-го типу на показники глікемії натще і постпрандіальної глікемії.
Матеріали та методи. Низькочастотна ультразвукова терапія проводилась 30 хворим із вперше виявленим ЦД 2-го типу, індекс маси тіла (ІМТ) яких перевищував 25 кг/м2. Ультразвуковий вплив здійснювався за допомогою апарата «МІТ-11» в імпульсному режимі 44 кГц при амплітуді коливань 2 мкм. Вплив на ділянку проекції печінки хворих виконувався за нерухомою методикою, експозиція на сеанс — 8 хв. Для посилення лікувального ефекту додатково впливали на сегментарні зони Th7-Th12 лабільною методикою по 2 хв на кожну ділянку. Курс лікування становив 10 сеансів. Хворі перебували на лікуванні дієтою, без застосування пероральних цукрознижувальних препаратів. Контрольна група складалася із 12 хворих із вперше виявленим ЦД 2-го типу, які мали надмірну масу тіла та яким була призначена традиційна терапія (дієта та фізичні навантаження). Ефективність низькочастотного ультразвука оцінювали з огляду на динаміку клінічних симптомів, глікемію та глюкозурію, рівень глікозильованого гемоглобіну (HbA1c). Для оцінки ступеня інсулінорезистентності визначали вміст інсулiну, С-пептиду і глюкагону у плазмі крові радіоімунологічним методом. Статистичну обробку результатів досліджень виконували з використанням t-критерію Стьюдента.
Результати. Початок лікувального ефекту спостерігався після двох процедур ультразвукового впливу. Максимальний ефект спостерігався після 8–10 сеансів лікування. Позитивна динаміка комплексного лікування полягала в поліпшенні загального самопочуття, зникненні астенізації, у зменшенні проявів серцево-судинних порушень, досягненні більш швидкої компенсації вуглеводного обміну. Курс лікування сприяв зниженню гіперглікемії у хворих із вперше виявленим ЦД 2-го типу. Рівень глікемії натще до лікування становив 10,3 ±
± 0,5 ммоль/л, після лікування — 5,00 ± 0,14 ммоль/л (Р < 0,001); постпрандіальна глікемія до лікування — 10,7 ± ± 0,5 ммоль/л, після лікування — 5,8 ± 0,2 ммоль/л (Р < 0,01). HbA1c до лікування становив 7,50 ± 0,08 %, після лікування — 5,8 ± 0,2 % (p < 0,01). Застосування низькочастотної ультразвукової терапії на ділянку проекції печінки у хворих із вперше виявленим ЦД 2-го типу сприяє підвищенню ефективності дії інсуліну на тканини і зниженню інсулінорезистентності. Низькочастотний ультразвук зменшує підвищену продукцію глюкози печінкою, сприяє нормалізації як глікемії натще, так і постпрандіальної глікемії. Незалежно від тривалості захворювання клінічне поліпшення супроводжується зниженням величини контрінсулінового гормону — глюкагону. Гальмуючи продукцію глюкози печінкою, ультразвукова терапія знижує глікемію натще, рівень HbA1c.
Висновки. Застосування низькочастотної ультразвукової терапії на шкірну проекцію печінки у хворих на ЦД 2-го типу сприяє нормалізації як глікемії натще, так і постпрандіальної глікемії. Вплив низькочастотним ультразвуком на проекцію печінки значно знижує показники, які характеризують інсуліносинтезуючу функцію підшлункової залози (імунореактивний інсулін, С-пептид) у хворих із вперше виявленим ЦД 2-го типу і з ІМТ > 25 кг/м2. Низькочастотний ультразвук знижує секрецію глюкагону і тим самим позитивно впливає на печінковий глюконеогенез. Ультразвукова терапія може використовуватись у комплексному лікуванні хворих із вперше виявленим ЦД 2-го типу.

Применяемая в настоящее время сахароснижающая терапия больных сахарным диабетом (СД) 2-го типа не всегда приводит к желательному результату, и очень много больных находится в состоянии постоянной декомпенсации обменных процессов с высокими показателями уровня гликемии и глюкозурии. Поэтому поиск оптимальной тактики лечения, которая могла бы повысить эффективность сахароснижающей терапии, уменьшить риск развития осложнений, остается одной из самых актуальных проблем современной диабетологии.
Цель исследования — изучить влияние низкочастотной ультразвуковой терапии на кожную проекцию печени у больных СД 2-го типа на показатели гликемии натощак и постпрандиальной гликемии.
Материалы и методы. Низкочастотная ультразвуковая терапия проводилась 30 больным с впервые выявленным СД 2-го типа, индекс массы тела (ИМТ) которых превышал 25 кг/м2. Ультразвуковое влияние осуществлялось с помощью аппарата «МІТ-11» в импульсном режиме 44 кГц при амплитуде колебаний 2 мкм. Воздействие на участок проекции печени больных выполнялось по неподвижной методике, экспозиция на сеанс — 8 мин. Для усиления лечебного эффекта дополнительно влияли на сегментарные зоны Th7-Th12 лабильной методикой по 2 мин на каждый участок. Курс лечения составлял 10 сеансов. Больные находились на лечении диетой, без применения пероральных сахароснижающих препаратов. Контрольная группа состояла из 12 больных с впервые выявленным СД 2-го типа, имевших избыточную массу тела, которым была назначена традиционная терапия (диета и физические нагрузки). Эффективность низкочастотного ультразвука оценивали с учетом динамики клинических симптомов, гликемии и глюкозурии, уровня гликозилированного гемоглобина (HbA1c). Для оценки степени инсулинорезистентности определяли содержание инсулина, С-пептида и глюкагона в плазме крови радиоиммунологическим методом. Статистическую обработку результатов исследований выполняли с использованием t-критерия Стьюдента.
Результаты. Начало лечебного эффекта наблюдалось после двух процедур ультразвукового влияния. Максимальный эффект наблюдался после 8–10 сеансов лечения. Положительная динамика комплексного лечения заключалась в улучшении общего самочувствия, исчезновении астенизации, в уменьшении проявлений сердечно-сосудистых нарушений, достижении более быстрой компенсации углеводного обмена. Курс лечения способствовал снижению гипергликемии у больных с впервые выявленным СД 2-го типа. Уровень гликемии натощак до лечения составлял 10,3 ± 0,5 ммоль/л, после лечения — 5,00 ± 0,14 ммоль/л (Р < 0,001); постпрандиальная гликемия до лечения — 10,7 ± 0,5 ммоль/л, после лечения —
5,8 ± 0,2 ммоль/л (Р < 0,01). HbA1c до лечения составлял
7,50 ± 0,08 %, после лечения — 5,8 ± 0,2 % (p < 0,01). Применение низкочастотной ультразвуковой терапии на участок проекции печени у больных с впервые выявленным СД 2-го типа способствует повышению эффективности действия инсулина на ткани и снижению инсулинорезистентности. Низкочастотный ультразвук уменьшает повышенную продукцию глюкозы печенью, способствует нормализации как гликемии натощак, так и постпрандиальной гликемии. Независимо от длительности заболевания клиническое улучшение сопровождалось снижением величины контринсулинового гормона — глюкагона. Тормозя продукцию глюкозы в печени, ультразвуковая терапия снижает гликемию натощак и уровень HbA1c.
Выводы. Применение низкочастотной ультразвуковой терапии на кожную проекцию печени у больных СД 2-го типа способствует нормализации как гликемии натощак, так и постпрандиальной гликемии. Влияние низкочастотным ультразвуком на проекцию печени значительно снижает показатели, которые характеризуют инсулинсинтезирующую функцию поджелудочной железы (иммунореактивный инсулин, С-пептид) у больных с впервые выявленным СД 2-го типа с ИМТ > 25 кг/м2. Низкочастотный ультразвук снижает секрецию глюкагона и тем самым положительно влияет на печеночный глюконеогенез. Ультразвуковая терапия может использоваться в комплексном лечении больных с впервые выявленным СД 2-го типа.

That is why the search for the optimal treatment strategy that could increase the effectiveness of hypoglycemic therapy, and reduce the risk of complications is one of the most actual problems of modern diabetology.

The continuous search for new methods of diabetes mellitus treatment is recently conducted. A variety of the physiotherapy methods of treatment are traditionally used in our country. The natural and modified physical factors can replace many methods of treatment. In most cases of chronic disease they become independent agents that have little or no side effects, and are different by low prices comparing with the pharmacological agents [1]. At present ultrasound is becoming more common together with hypoglycemic agents for improving their efficiency.

Ultrasound therapy involves the application of mechanical energy fluctuations of an elastic medium with a frequency above 20 kHz. In recent years the low-frequency ultrasound is beginning to be used in medical practice along with the high-frequency ones. It exceeds the physiological and therapeutic effects of the high-frequency ultrasound [2]. Low-frequency ultrasound has a universal therapeutic effect, consisting of mechanical miromassage of the cells and the tissues, energy influence of ultrasonic waves on the body tissues, and the physical and chemical actions. The biological effects of ultrasound on the body is to increase an activity of the adaptive and protective mechanisms. The therapeutic effect is expressed as anti-inflammatory, spasmolytic, desensitizing effects, and the stimulation of regenerative processes [3]. The human body and its internal organs are acoustically «opaque» for the low-frequency ultrasound, which makes it possible to influence them through the skin areas to which it is projected. The deep-seated human internal organs are voiced with the low-frequency ultrasound (effect depth — 10–20 cm) [4].

Currently, the most perfect device of low-frequency ultrasound therapy is the apparatus MIT-11. An ultrasonic impact is performed in a pulse mode of 44 kHz and the amplitude of fluctuations — 2 microns. The pulse mode consists in greater softness and the duration of manifested effects. The softness of action is associated with the predominance of physical and chemical influences over the thermal and mechanical ones. This type of impact is most suitable when scoring the endocrine formations sensitive to ultraacoustic energy. The pulse mode is directed to restoring synthesis [6].

It is known that the key factors in the development of hyperglycemia in type 2 diabetes are: functional insufficiency of -cells causing a decrease in insulin secretion; cell insulin resistance and the inability to use glucose as an energy substrate; an increase in hepatic glucose production. The liver is one of the major insulin-sensitive organs involved in carbohydrate metabolism. Affecting with low-frequency ultrasound on the liver, we are acting on the basic links of disease pathogenesis [5].

The purpose of this study was to determine the use of low-frequency ultrasound therapy on cutaneous projection of the liver in patients with type 2 diabetes mellitus.

Materials and Methods

30 patients with newly diagnosed type 2 diabetes mellitus and body mass index (BMI) greater than 25 kg/m2 were treated by the low-frequency ultrasound therapy. Clinical studies were performed with the consent of patients. The ultrasonic effects were conducted in pulsed mode of 44 kHz and an amplitude of fluctuations — 2 microns using the apparatus MIT-11 as follows. Effects on the area of liver projection in patients was performed by the immovable technique (method), exposure to a session consisted of 8 minutes. The vaseline oil was used as a contact substance. Additionally influence on segmental area of Th7-Th12 by labile technique for 2 minutes on each plot was used to enhance the therapeutic effect. The course of treatment was 10 sessions. The patients were on a diet, without the use of hypoglycemic agents. The control group consisted of 12 patients with newly diagnosed type 2 diabetes mellitus and excessive body weight (BMI  > 25 kg/m2) and who were appointed to conventional (traditional) therapy (diet and exercise).

The patients were examined before and after treatment. The dynamics of clinical symptoms, glycemia and glycosuria, level of glycated hemoglobin was taken into account to assess the effectiveness of low-frequency ultrasound. To assess the degree of insulin resistance content of insulin, C-peptide and glucagon in blood plasma were determined by radioimmunoassay. Fasting insulin, C-peptide, glucagon in blood serum were tested in the morning. The analysis was conducted using the kit for immunoradiometric determination — IMMUNOTECH (Czech Republic). The determination of the insulin level in blood plasma was perform at the same time for patients with type 2 diabetes mellitus and control group with one kit of reagents, which allows to minimize the error of the data obtained.

The statistical processing of tests’ results was performed using Student’s t test.

The Results and Their Discussion

The beginning of therapeutic effect was observed after 2 procedures of the ultrasound exposure. The maximum effect is appeared after 8–10 treatment sessions. The positive dynamics of complex treatment is improving the general state of health, a disappearance of asthenization, and a decrease in the symptoms of cardiovascular disorders, achieving faster compensation of carbohydrate metabolism.

The course of treatment contributed to the hyperglycemia reduction in patients with newly detected type 2 diabetes mellitus.

Fasting glycemia level in patients with newly diagnosed diabetes mellitus before treatment was 10.3 ± 0.5 mmol/l, after treatment — 5.00 ± ± 0,14 mmol/l (P < 0.001), postprandial glycemia before treatment was 10.7 ± 0.5 mmol/l, and after treatment — 5.8 ± 0.2 mmol/l (P < 0.001). Glycated hemoglobin before treatment consisted of 7.50 ± 0.08 %, after treatment — 5.8 ± 0.2 % (P < 0.01).

Fasting glucose level (glycemia) before treatment was 11.2 ± 0.65 mmol/l after treatment — 9.2 ± ± 0.6 mmol/l (P < 0.05), postprandial glycemia before treatment was equal to 12.6 ± 0.25 mmol/l after treatment — 10.40 ± 0.04 mmol/l (P < 0.01) in the control group. Glycated hemoglobin before treatment was 7.50 ± 0.08 %, after treatment — 6.4 ± 0.1 % (P < 0.05).

We noted a significant decrease in the indices characterizing pancreatic function (Table 1).

A normalization of the parameters characterizing the synthesizing insulin function of the pancreas is not observed during the traditional treatment (Table 2).

The use of low-frequency ultrasound therapy to the area of liver projection in patients with newly diagnosed type 2 diabetes mellitus contributes to improve the efficiency of insulin action on tissues (reducing insulin resistance). Low-frequency ultrasound reduces the increased hepatic glucose production, helps to normalize both fasting plasma glucose (glycemia) and postprandial glycemia. The clinical improvement is associated with decreasing counter insulin hormone — glucagon levels, regardless of disease dura-tion.

The data obtained point out an increased efficiency of insulin action on the tissues, suggest the possibility of reducing insulin resistance in patients with newly diagnosed type 2 diabetes mellitus and BMI > 25 kg/m2. Inhibiting hepatic glucose production, the ultrasound therapy lowers the fasting glucose and the level of glycosylated hemoglobin.

After ultrasound treatment, the authors noted a positive dynamics of clinical symptoms: an improvement of the general health status, a decrease in fatigue, an improvement of psycho-emotional indices, disappearance of pain in the right upper quadrant, and a decrease in liver size in all the patients under study.

Conclusions

1. The use of low-frequency ultrasound therapy on cutaneous projection of the liver in patients with type 2 diabetes mellitus promotes the normalization both fasting and postprandial glycemia.

2. The effect of low-frequency ultrasound on cutaneous projection of the liver is significantly decreasing parameters that characterize the pancreatic insulin synthesizing function (IRI, C-peptide) in patients with newly diagnosed type 2 diabetes mellitus and a BMI  > 25 kg/m2.

3. Low-frequency ultrasound reduces the glucagon secretion and thereby positively affects the hepatic gluconeogenesis.

4. Ultrasound therapy can be used in the complex treatment of patients with newly diagnosed type 2 diabetes mellitus.
