The system of psychoprophylaxis of the repeated episodes of the recurrent depressed disorder

Recurrent depressive disorder has high medical and social significance because of the high risk of implementation of self-destructive modes of behaviour and suicident in patients suffering from mental disorders in the framework of this nosological form.

Despite the high prevalence of this disorder, the problem of timely clinical identification and rational psychological prevention of repeated episodes of recurrent depressive disorder remains unsolved.

In our view, deciding this problem is possible by creation of the system of psychological prevention that will be conducted in the prodromal period of the recurrent depressed disorder.

Prodromal period of recurrent depressive disorder has its polymorphic psychopathological disorders, which often subjectively not identified by patients as painful. In this period it is appropriate to teach patients to the self-diagnose.

The purpose of self-diagnosis is the timely appeal of patients for specialized medical help, even before the development of repeated episodes of recurrent depressive disorder and is the preventing the exacerbation of the disease.

In this regard, the aim of the study was to develop a system of psychological prevention, which would prevent recurrent depressive disorder under condition of its use in the prodromal period of disease in the period of availability of the predictors of recurrent depressive disorder.

The study was conducted based on the Municipal medical institution «Municipal psychoneurological hospital №2», Donetsk. The study involved 100 patients with recurrent depressive disorder, which was observed for three years. All patients were men. The average age of the patients was 45.5±1.50 years.

To establish the characteristics and dynamics of recurrent depressive disorder was used anamnestic, clinical-psychopathological, psychodiagnostic, statistical methods.

The study was developed methodological basis of the system of psychological prevention, aimed at preventing the exacerbation psychopathological disorders in the framework of recurrent depressive disorder in the period of the prodromal period of repeated episodes.

Under the proposed system of psychological prevention, the implementation of the therapeutic effect was due to several vectors:

– autodiagnostic (autonosognostic), with the following timely appeal to medical institutions;

– medical-prophylactic, as an option to build or renewal of compliance;

– autotherapeutic, as a variant of auto-behavior modification.

Educate patients used the following psychotherapeutic technique: behavioral, cognitive, including behavioral therapy Beck; autosuggestion Schultz.

In the result of the introduced system of psychological prevention in patients of the main group there was total reduction of psychopathological symptoms in 84.00 % cases, moderate improvement - in 14.00 %. Just a depressive episode was avoided in 98.00 % of cases. Significant positive dynamics was not detected in 2 % of cases; in these patients a month has developed depressive disorder.

The results of the research prove that the system of psychological prevention is effective, its application in the prodromal period prevented the development of re depressive episodes in most patients suffering from recurrent depressive disorder.
