Logistical approach to management of material reserves of disaster medicine service in the provision of emergency medicalcare

Solution to the problem of optimal resource provision of disaster medicine service allows to organize an effective system of medical evacuation support, to provide emergency medical care to victims in pre-hospital and in-hospital period in a timely manner, to implement sanitary and anti-epidemic measures based on current legislative and regulatory framework of Ukraine. The purpose of this work is scientific substantiation of expediency of using the modern paradigm of logistics and its approaches to optimization of resource support in Disaster Medicine Service activities. Research materials include basic material and information flows of activity of the Service (forces, resources, medical transport, victims, medical and non-medical staff) in everyday conditions and under the elimination of sanitary losses in emergencies. The methods applied in the study are system approach, logistic analysis, information and analytical method and the method of scientific paradigmal extrapolation.

Application of the logistic achievements in the process of organizing and providing of emergency medical assistance to victims of emergencies lies in organization of a rational promotion process of resources from manufacturers and suppliers to the Disaster Medicine Service, in organization of functioning of circulation resources area and management system, in formation of infrastructure of Disaster Medicine Service. The concept of logistics system and a list of its components are defined. The objectives of certain types of logistics which improve the organization of resource provision of medical care to victims in emergencies are characterized. The role of logistics information flow in the Service is described. The main tasks of logistics and advantages of its use are formulated.

The novelty of logistics capabilities in optimization of organization of the Disaster Medicine Service activities lies in shifting priorities among the various economic activities in favor of strengthening the importance of the management of material flows. Flows emergencies are taken for the main, counter flows of the organization and provision of emergency medical care, life support and logistical resources are secondary. The system of science-based views on the optimization of Disaster Medicine Service activities and improvement of the organization of emergency medical care to victims by rationalizing management of material flow is the concept of logistics of Disaster Medicine Service. Development of theoretical foundations of logistically organized emergency medical care using the medical and hospital technologies of modern medicine and scientifically based recommendations for their practical use by the Disaster Medicine Service are important scientific and practical direction in population protection.

Logistically constructed health-care provision in emergency situations is carried out in the form of science-based activities, taking into account the guiding principles: a) systematicity and comprehensiveness relying on forces and resources of management system of Disaster Medicine Service of Ukraine; b) interaction with the forces of civil protection, military medicine and medical institutions of all ownership forms; c) a high degree of health workers' ability to adapt to environmental changes in emergencies. Methodology of logistics management system of Disaster Medicine Service is to strengthen the technical and technological, economic and functional coherence of the participants which provide emergency medical assistance to victims in emergency situations. The basic premise of the possibility of applying the method of logistic organization of emergency medical care to victims in activities of disaster medicine service lies in a clear organizational and economic unity of the participants and the development of highly efficient medical and technical systems, which are able to provide the necessary emergency medical care in the right place, at the right time, at the right volume and at minimal cost.
