Experimental Study of Osteosynthesis Types in Midclavicular Fractures

The Laboratory of Biomechanics SI "IPHS from Prof. MISytenko NAMS " we have conducted experimental research strength osteosynthesis of the clavicle through various means. The experiment was carried out on plastic copies of the clavicle, which, by cutting, modeled two types of direct-to- oblique fractures. Osteosynthesis was carried out using intramedullary rod nakistkovoyi plate and developed by us intramedullary device for the osteosynthesis of the clavicle.

We studied displacement of the clavicle fragments under the influence of bending stress. The load was applied to the model in two directions: top- down and fron-back.

In total, the experiment used 36 models clavicle (3 for each model variant modeling).

The experiment was performed as follows. Proximal clavicle model rigidly fixed to the stand, to the distal end of the applied bending force. The value of bending strength gradually increased from 0 to 50 N. The magnitude of displacement of the distal end of the fixed discrete, the corresponding values bending force of 10 N. Step

These experiments were processed statistically. The data refer to the metric (deflection fragments). For each type of fixation of the fragments were obtained for three values of the displacement at a fixed effort that was applied. Comparative analysis of the difference series for the same type of clamps in various types of fracture and the direction of the load was carried out using a paired Due to the fact that compared three types of fixation of fragments, was used to compare single-factor analysis of variance, and comparisons used for each value of the effort that applied .

In the vertical load, both in direct and in oblique fractures, nakistkova plate apparatus and our design showed almost identical fixing properties. Indicators of the dislocation debris in the osteosynthesis rod intramedular some worse, but the difference does not exceed 1.5 mm for all the load . This difference is explained by the presence of rotational component of the deformation.

Under the influence of the horizontal load intramedullary rod and our machine designs behave roughly the same way. Much worse than the horizontal load is nakistkova plate. The value of the displacement of fragments is much higher than for intramedullary rod and the unit of our design. For loads acting in the plane of the overlay plate, the plate is practically not working. All load falls on the fixing screws, most of which falls on the first screw from the point of application of load.

Designed by our apparatus for osteosynthesis of the clavicle , with different directions of loading and behaves as intramedullary rod and plate as combining their positive qualities. Designed by our device is based load equally in all directions, similarly intramedullary rod. However, due to the presence of fixing screws, no rotational component of deformation, which is typical for nakistkovoyi plate.
