The receptor basis of the mechanism of development of local inflammation of the mucous membrane of the stomach in children with helicobacter pylori infection

В статье приведены данные о роли факторов рекогниции врожденного иммунитета в развитии локального воспаления слизистой оболочки желудка у детей с хеликобактерной инфекцией. Целью исследования явилось изучение роли механизмов распознавания липополисахарида Helicobacter pylori (Н.pylori) в развитии локального воспаления слизистой оболочки желудка у детей с хеликобактерной инфекцией. Под наблюдением находилось 55 детей в возрасте от 10 до 16 лет, больных хроническим Н.pylori-ассоциированным гастродуоденитом (28 пациентов, основная группа) и хроническим гастродуоденитом, не ассоциированным с Н.pylori (27 детей группа сравнения), в стадии обострения. Показано, что в основе развития инфекционно-воспалительного процесса, вызванного Н.pylori, и его неблагоприятного течения лежит функциональный дефицит неспецифических механизмов защиты желудочно-кишечного тракта. Приведены особенности экспрессии Toll-подобного рецептора 4 (TLR4) в биоптате слизистой оболочки желудка и содержания протеина sCD14 в сыворотке крови детей, больных хроническим гастродуоденитом в стадии обострения. Показано, что развитие у детей хронического гастродуоденита, ассоциированного с Н.pylori, сопровождается повышением экспрессии в биоптате слизистой оболочки желудка TLR4, который является ключевым сенсором для липополисахарида Н.pylori, и низким уровнем TLR4-аксессуарного протеина sCD14.

У статті наведені дані про роль факторів рекогніції вродженого імунітету в розвитку локального запалення слизової оболонки шлунка в дітей із хелікобактерною інфекцією. Метою дослідження було вивчення ролі механізмів розпізнавання ліпополісахариду Helicobacter pylori (Н.pylori) у розвитку локального запалення слизової оболонки шлунка в дітей із хелікобактерною інфекцією. Під спостереженням перебували 55 дітей у віці від 10 до 16 років, хворих на хронічний Н.pylori-асоційований гастродуоденіт (28 пацієнтів, основна група) та хронічний гастродуоденіт, не асоційований з Н.pylori (27 дітей, група порівняння), у стадії загострення. Показано, що в основі розвитку інфекційно-запального процесу, викликаного Н.pylori, і його несприятливого перебігу лежить функціональний дефіцит неспецифічних механізмів захисту шлунково-кишкового тракту. Наведені особливості експресії Toll-подібного рецептора 4 (TLR4) в біоптаті слизової оболонки шлунка і вмісту протеїну sCD14 у сироватці крові дітей, хворих на хронічний гастродуоденіт у стадії загострення. Показано, що розвиток у дітей хронічного гастродуоденіту, асоційованого з Н.pylori, супроводжується підвищенням експресії в біоптаті слизової оболонки шлунка TLR4, який є ключовим сенсором для ліпополісахариду Н.pylori, і низьким рівнем TLR4-аксесуарного протеїну sCD14.

The article presents data on the role of factors for innate immunity recognition in the development of local inflammation of the gastric mucosa in children with Helicobacter pylori (Н.pylori) infection. The aim of the study was to investigate the role of mechanisms of H.pylori lipopolysaccharide recognition in the development of local inflammation of the gastric mucosa in children with Н.pylori infection. We observed 55 children between the ages of 10 to 16 years with chronic H.pylori-associated gastroduodenitis (28 patients, study group) and patients with chronic gastroduodenitis non-associated with H.pylori (27 children, comparison group) in the acute stage. It is shown that the basis of an infectious-inflammatory process caused by Н.pylori, and unfavorable clinical course is functional deficiency is non-specific mechanisms of protection of the gastrointestinal tract. Features of expression of Toll-like receptor 4 (TLR4) in the biopsy sample of the gastric mucosa and the protein sCD14 content in blood serum are demonstrated for children with chronic gastroduodenitis in the acute stage. It is shown that the development of chronic gastroduodenitis associated with Н.pylori in children is accompanied by increased TLR4 expression in biopsy specimens of gastric mucosa, which is a key sensor for Н.pylori lipopolysaccharide, and low levels of TLR4-accessory sCD14 protein.

Introduction: value of the factor of Helicobacter pylori (H. pylor)i in the development of gastrointestinal disease, is generally accepted. Response against H. pylori depends on the state of immunity, the number of receptors on the gastric mucosa contributing to the microorganism adhesion, H. pylori virulence and others. Discovery of the molecular mechanisms of nonspecific innate immunity (Toll-like receptors (TLR) and TLR-accessory molecules), clarified the meaning of the latter in the development of H. pylori infection in children. TLR4  plays a special role in the development of infectious-inflammatory process caused by gram-negative infectious agents including H. pylori. Receptor TLR4 is a key sensor for LPS. Purpose: to investigate the role of mechanisms of H. pylori lipopolysaccharide recognition in the development of local inflammation of the gastric mucosa in children with H. pylori infection.

Material: 55 children aged from 10 to 16 years with chronic H. pylori-associated gastroduodenitis (28 patients, study group) and patients with chronic gastroduodenitis nonassociated with H. pylori (27 children group) in the acute stage.

Methods: endoscopy of the esophagus, stomach and duodenum, was conducted for all children according to the standard technique of biopsy mucosa of antrum and immunological investigations that reflect the level of interest of H. pylori infection in the pathogenesis of the disease. Method of enzyme-linked immunosorbent assay (ELISA) was used to estimate the concentration of serum sCD14, (ELISA test kit, Diaclone, France). TLR4 gene expression level was determined by polymerase chain reaction (PCR) in real time (Real-time) in the Institute of Genetic and immunological basis of disease and pharmacogenetics (MD, PhD I.P.Kaydashev, Poltava).

Results. Research results TLR4 expression in the biopsy of the gastric mucosa (GM) and the level  of sCD14 (ng / ml) in the serum of children with chronic gastroduodenitis showed that the expression of TLR4 in the biopsy of the gastric mucosa children with chronic gastroduodenitis associated with H. pylori, was increased compared with children chronic gastroduodenitis nonassociated with H. pylori (1,59 ± 0,12 and 1,28 ± 0,09 respectively, Pu <0,05). The  level of concentration of sCD14 was significantly increased in the serum of children with chronic gastroduodenitis nonassociated with H. pylori, compared with a group of children with chronic gastroduodenitis associated with H. pylori (15033,3 ± 1531,3 and 4009,5 ± 401,8 respectively, Pu <0,05).

Conclusions. Thus, the heart of development of infectious-inflammatory process caused by the H. pylori, and adverse course is functional deficiency nonspecific defense mechanisms of the gastric mucosa. The development of the inflammatory process in the gastric mucosa of children with chronic gastroduodenal pathology depends on the interaction of H. pylori LPS as its key the sensor TLR4, and with TLR-accessory protein sCD14. Sufficient level of sCD14 reduces the severity of LPS-associated inflammatory effects H.pylori. With low concentrations of sCD14 LPS H. pylori interacts with mCD14, leading to the formation of LPS/TLR4/MD-2, which activates the inflammatory process in the gastric mucosa. Considering a protective role in the innate immune response and TLR-TLR accessory molecules involved in the recognition of LPS H. pylori, in particular, sCD14, a new direction in the treatment of Helicobacter pylori infection in children may be drug activity regulation TLR4 and products sCD14.
